In total thyroidetomy both lobes as well as the isthumus and surrounding lymph nodes are usually removed. In lobectomy only one lobe is removed. In sub total thyroidectomy the isthumus(which connects the 2 lobes) and part of the other lobe is removed. In most cases except cancer, care is taken to avoid removing the parathyroid glands or damaging them . If it is to be removed calcium supplements are a must .As far as the laryngeal nerve damage is concerned, only remote chances of damage occur in experienced hands.
